New Patients

In order to register as a new patient, you must complete the following forms on our secure site. You may print the forms and bring them to our office or submit the forms online. Click the link below to access forms.

FINANCIAL POLICY

Susan M. Cable, DDS requires payment in full at time of service.

We accept cash, check, Care Credit, and all major credits cards.

We offer a 5% courtesy accounting adjustment to patients who do not have insurance if they pay by cash or check in full at time of service.

If you choose to discontinue care before treatment is complete, you will receive a refund less the cost of the care received.

Treatment plans that require multiple appointments may require a down payment.

A finance charge of 18% interest (1.5% per month) will be applied annually to all past due accounts.

A fee of $30 is charged to patients who miss or cancel more than one time in a calendar year without 24-hour notice.

Susan M. Cable, DDS charges $30 for returned checks.

Our office is an unrestricted provider. If you have questions about what your insurance plan will cover at our office or financials, please call and our insurance specialists will be happy to assist you so you can receive the treatment you need. We encourage you to know your insurance benefits.
